- 0.6 ≤ vertical cup/disc ratio (CDR) < 0.8 OR diffuse neuretinal rim (NRR) thinning OR 0.2 ≤ CDR asymmetry < 0.3 OR violated ISNT rule–all without GVFD.
- GVFD without glaucomatous ONH changes.

- Diffuse NRR thinning with CDR ≥0.8 regardless of visual field (VF).
- CDR ≥ 0.6 or diffuse NRR thinning (regardless of CDR) with GVFD.
