- Physical examination and introduction of the fistuloscope (8° lens, 3.3 × 4.7 mm, Karl Storz GmbH, Tuttlingen, Germany) through one of the fistula openings after its cruentation if necessary. In the latter case, we use a 4 mm skin punch (Figure 3).
- Identification of the main sinus and of possible secondary tracts.
- Removal of the hairs with endoscopic grasper introduced through the fistuloscope working channel.
- Brushing of inflammatory and necrotic tissue, obtained with an endoscopic brush.
- Thermocoagulation of the cavity’s walls with an endoscopic monopolar electrode.
