Special Issue Information

Dear Colleagues,

In recent decades, the high prevalence and morbidity of digestive diseases has led to the development of more efficient endoscopic visualization and instrumentation for gastrointestinal tract examination, as well as of the biliary tree and of the pancreas. Advances in optical physics and bioengineering, driven by the push for higher diagnostic yields and personalized treatment, are revolutionizing diagnostic and therapeutic endoscopy. The use of endoscopic ultrasound (EUS) has evolved beyond merely differentiating tissue densities. With modalities like elastography, the optimization of spectral and color Doppler imaging, tissue features, and vascularity can be characterized in detail. These techniques allow for precise biopsy targeting, facilitating microscopic examination or molecular characterization to ensure optimal diagnosis and rapid referral to therapy. The field of hepatology is rapidly expanding with EUS, combining endoscopy and sonography in one hybrid device that can be brought close to the liver. The field of endo-hepatology is finding itself at the frontier of interventional EUS. Managing pancreatic lesions, particularly pancreatic cancer, remains challenging due to poor prognosis. EUS plays a crucial role in the diagnosis, staging, and treatment of pancreatic lesions, with potential future research aimed at revolutionizing personalized patient care. Endoscopic retrograde cholangiopancreatography (ERCP) has well-known therapeutic applications. Novel imaging modalities like endomicroscopy and collecting bile for molecular analysis using next-generation sequencing of bile cell-free DNA (cfDNA) have the potential to increase the diagnostic yield of ERCP. Obesity, a chronic multifactorial disease linked to numerous cardiometabolic conditions, requires a multidisciplinary approach for successful management, including lifestyle modifications, pharmaceutical therapies, and bariatric surgery. Endoscopic bariatric therapies have emerged as a promising bridge between medical management and bariatric surgery. This Special Issue highlights emerging technologies in endoscopic imaging, tissue acquisition, and analysis, which are currently enhancing diagnostic yields and shaping the therapeutic approach to gastrointestinal and biliopancreatic diseases.
