Thank you for the pleasure of reviewing the manuscript titled "Target Heterogeneity in Oncology: the Best Predictor for Differential Response to Radioligand Therapy in Neuroendocrine Tumors and Prostate Cancer". It is very well written, organized, and detailed.
I find no concerns that need to be addressed as it relates to the content or presentation of this manuscript.
There a few minor grammatical issues that the authors need to fix. They are:
- Line 556: "wascombined" requires a space to be "was combined"
- Line 564: is "et" after "MRI" a typing error?
- Line 584: "In both study" needs to be "In both studies"
- Lines 635 - 636: "assessed noninvasively assessed using" needs to be "assessed noninvasively using" or "noninvasively assessed using"
- Line 694: The sentence sounds awkward. Instead of "Although not remarkable high an Area..." perhaps "Although not remarkably high, an Area..."
Finally, please double check to make sure all abbreviations and acronyms have been spelled in completely prior to their first use.

This review very elaborately described the target heterogeneity in neuroendocrine and prostate cancers and the consequent differential response to radioligand therapy. They nicely introduced the radioligand therapy with sufficient background. They introduced the general concepts of tumor heterogeneity, elaborated on in vitro, serum based, liquid biopsy and pharmaco-genomics based markers for identifying such heterogeneity. They have detailed on radioligand therapy principles and discussed the very recent developments in the field. Finally, they have discussed the effects of such heterogeneity on the outcome of the radioligand therapy. They have also discussed the future perspectives in the field. The entire review is supplemented with nice cartoons to explain the various features of the radioligand therapy and the target heterogeneity. They have summarized a substantial set of information in the tabular format. This review is extremely informative and a very good read for the beginners in the field as well as useful for the experts to gain sufficient in-depth knowledge. This reviewer has no hesitation in recommending publication of this review in the present format.
