Special Issue Information
Dear Colleagues,
Health services have made fantastic advances in promoting the integration and sustainability of novel good practices and inter-sectorial approaches to health promotion and disease prevention and integrated care. We as a society have accumulated a great deal of knowledge on effective ways to treat health problems, but the actual knowledge on how to implement good practices is still lagging. Best-suited good practices or recommended interventions are being tested to identify the practices or interventions that fit patient needs and local context. New findings highlight the importance of a systems approach not only to safer primary care for people with chronic conditions and frailty, but also to secondary and tertiary care, for the improvement of work access and participation of people with higher needs. This critical situation requires a complex and unified response to today’s challenges, but identified good practices can be converted into real life with a direct benefit for patients.
To have are a real and significant impact on better health care management, guidance for health sector stakeholders is a must. We all seek a Europe that is free of preventable diseases, premature death, and avoidable disability. This would be possible if successors that have been showing steep improvements could share their practical experience, providing opportunities for others to learn from their success.
For this Special Issue we propose to include manuscripts that address the following topics: practical innovations of health care systems, agencies, and organizations established to transfer/implement good practice in order to improve the health outcomes of the communities they serve, with a focus on potential good practices in areas like multi-morbid patients’ quality of care; health promotion and primary prevention; tackling functional decline and quality of life as the main; and healthcare management making health systems sustainable and responsive to the aging of our population.
This Special Issue seeks papers on good practice transfer, including those that explore the impact of interventions at all stages of the life cycle, with special attention to the heaviest health care resource users and the most vulnerable subgroups of the population, considering both experimental and non-experimental designs. Systematic reviews and meta-analyses will also be considered.

Submitted manuscripts should not have been published previously, nor be under consideration for publication elsewhere (except conference proceedings papers). All manuscripts are thoroughly refereed through a single-blind peer-review process. International Journal of Environmental Research and Public Health is an international peer-reviewed open access monthly journal published by MDPI.
